What we do:

Referral – Our Case Managers will help you find the best community resources to meet your needs.

Linkage – We will help you connect to identified community resources.

Advocacy – Our Case Managers will teach you how to effectively communicate with these resources to ensure that you receive quality services.

Monitoring – We will monitor your progress as you access services and as your needs change we will ensure you continue to be connected with the community resources that best fit your needs.

Who are We?

Our bilingual team is made up of professionals, who are immigrants like you, or they were born and/or have been living in another country. We understand how difficult it can be to adapt to a new place and culture. Our team understands your needs and we will work with you to find a better way to reach your dreams and/or life goals.

Within 48-72 hours, one of our bilingual professionals will call you to set up an initial interview to discuss the best services to meet your needs.

How are services paid for?

Anyone in the family household with Soonercare/Medicaid can receive services. Call us for more information @ 918-712-0859. If you do not have Soonercare, but you are interested in receiving services, we have other alternatives for you.
